Job Description:

A company is looking for a Continuous Care Coach to manage the care needs of patients with chronic eye diseases.

Key Responsibilities

Assess, coordinate, and manage patient care needs
Develop and implement personalized care plans in collaboration with patients and healthcare providers
Engage with patients and their families to provide support and education throughout their care journey

Required Qualifications

2+ years of work experience in eye care
Certified Ophthalmic Technician (COA) or Certified Ophthalmic Technician (COT/COMT)
Proficient with Electronic Medical Record (EMR) systems, preferred
Ability to work in a team environment with minimal supervision
Must have a dedicated remote workspace and reliable internet access
